1. Acceptance of Terms
Welcome to MySamInsurance.com (the “Site,” “we,” “us,” or “our”). These Terms of Service (“Terms”) govern your access to and use of the Site, including any request for an insurance quote submitted through the Site. By accessing the Site, submitting a form, requesting a quote, or otherwise providing your contact information, you (“you,” “User,” or “Prospect”) acknowledge that you have read, understood, and agree to be bound by these Terms, including the consent to contact provisions described in Section 3 below.
If you do not agree to these Terms, please do not use the Site or submit any information through it.
2. Description of Service
The Site allows Users to request SAM insurance quotes by submitting personal and contact information through an online form. Quote requests submitted through the Site are reviewed and facilitated by licensed insurance agents affiliated with Paperless Insurance Services, Inc. (the “Agency”). The Site itself does not issue insurance policies; all policy issuance, binding, and servicing is handled by a duly licensed insurance agent or the applicable insurance carrier.
3. Consent to Be Contacted (Please Read Carefully)
By submitting your contact information through the Site – including but not limited to your phone number and email address – and requesting an insurance quote, you expressly consent to be contacted by or on behalf of the Agency regarding your quote request and related insurance products and services.
Specifically, you agree and acknowledge that:
3.1 Email Communications. You consent to receive emails from the Agency at the email address you provide, including quote information, follow-up communications, and related insurance materials.
3.2 Automated Telephone Contact – Initiation Only. You consent to receive telephone calls initiated using an automated telephone dialing system and/or artificial or prerecorded voice technology (“Automated Voice Service”) at the telephone number you provide, for the purpose of initiating contact regarding your quote request.
- The Automated Voice Service is used solely to initiate and route your call.
- Once your call is initiated, it will be connected/routed to a duly licensed, live human insurance agent for the purpose of discussing, reviewing, and facilitating your insurance quote. The Automated Voice Service is not used to deliver a fully automated sales pitch, collect payment, or bind coverage.
- You are required to provide your accurate contact telephone number, as required by insurance carrier to underwrite/issue/service your policy and claims. Providing accurate contact information is necessary in order for the Agency to respond to and facilitate your specific quote request.
3.3 Consent Is Not a Condition of Purchase. Your consent to receive automated calls or emails is not required as a condition of purchasing any insurance product or service. You may still request a quote by contacting the Agency directly through alternative means listed in Section 8.
3.4 Message and Data Rates. Message and data rates may apply to communications sent to your mobile device, depending on your carrier and plan.
3.5 Revoking Consent. You may revoke your consent to receive automated calls, texts, or emails at any time by:
- Replying “STOP” to any text message received;
- Clicking “unsubscribe” in any email received;
- Verbally informing an agent during a call that you wish to be placed on a do-not-call list; or
- Contacting us directly using the information in Section 8.
Revoking consent will not affect any insurance policy already in force but may limit the Agency’s ability to follow up on a pending quote request.
4. Accuracy of Information
You agree to provide accurate, current, and complete information when requesting a quote. Insurance quotes are estimates based on the information provided and are subject to underwriting review, verification, and approval by the applicable insurance carrier. A quote is not a binder, policy, or guarantee of coverage.
5. No Insurance Advice; No Guarantee of Coverage
Information provided on the Site is for general informational purposes only and does not constitute insurance, legal, financial, or professional advice. Final coverage terms, premiums, and eligibility are determined solely by the underwriting insurance carrier and your Insurance Policy document.
6. Privacy
Your use of the Site is also governed by our Privacy Policy, available at https://mysaminsurance.com/privacy-policy/, which describes how we collect, use, and protect your personal information, including information used to facilitate the contact described in Section 3.
7. Limitation of Liability
To the fullest extent permitted by law, the Agency shall not be liable for any indirect, incidental, special, or consequential damages arising out of your use of the Site or your quote request, including any delay or failure in contacting you.
